& Trust, NA. By using Internet technology, CapWest Mortgage Corporation and/or Farmers Bank & Trust, NA can give
you an approval in minutes, including important federal and state law disclosures. To do this, we need you to
consent to our giving you these disclosures electronically.
This document informs you of your rights when receiving legally required disclosures, notices and information
("Disclosures") electronically from CapWest Mortgage Corporation and/or Farmers Bank & Trust, NA.
Electronic Communications
To better serve you, certain Disclosures related to your Request may be provided to you electronically via email.
If you choose this mode of delivery, the information sent will not be encrypted and will be sent over an insecure
Internet connection. Most of the information to be provided is statutory and is not personal to you. The only personal
information sent will consist of your name, loan amount, interest rate, and payment amount. None of the other
information provided during the application process, such as your social security number, address or specific
personal financial information will be provided electronically.
Your Consent.
By proceeding, you consent to receive limited Disclosures electronically. Most Disclosures will be sent by U.S.
Mail within 3 business days. If you do not wish to consent to electronic delivery of disclosures, but wish to continue
with your application, please call the home office at the following toll-free number: 866-280-WEST (9378).
You understand that you can withdraw your consent at any time and at no cost to you by calling 866-280-WEST (9378).
Scope of Consent.
Your consent to receive Disclosures and to do business electronically, only applies to your use of this site or our
Mortgage Center and your application for a mortgage loan with CAPWEST . This consent does not extend to the closing
of your loan with us nor to any other parties involved in the transaction such as appraisers, title agents or attorneys.
Withdrawing Consent.
Because CAPWEST may provide certain Disclosures to you as soon as you consent, you will not be able to withdraw your
consent to receive those Disclosures electronically. However, you may request paper copies of those disclosures
(see Requesting Paper Copies below). You can also withdraw your consent to receive further Disclosures electronically
at any time and at no cost to you. You may do so by contacting us at 866-280-WEST (9378). If you decide to withdraw
your consent, the legal validity and enforceability of prior electronic Disclosures will not be affected.
Printing or Saving Electronic Documents.
If you choose electronic delivery and want to retain the Disclosures (including this notice) to read them later,
you will need to print them or save them. To print them, you'll need a basic printer capable of printing Web pages.
To save any of the disclosures, you can use the "Save" feature of your Internet browsing software to save the pages
to your hard drive or to some other media, such as a floppy disk. Requesting Paper Copies If you wish to obtain a
paper copy of any of the Disclosures that are being provided electronically, you can call us at 866-280-WEST (9378)
and we will send paper copies to you at no charge.
Required Hardware and Software.
Before you decide to do business with us electronically, you should check to make sure you have the necessary hardware
